& Privacy Policy

The Service

SportFormIQ ("we," "us," "our") provides a web-based player evaluation platform for youth soccer clubs ("the Service"). The Service allows clubs to import player rosters, conduct structured evaluations using internationally-aligned frameworks, record coaching decisions, and export results.

By accessing or using the Service you agree to be bound by these Terms of Service. If you are using the Service on behalf of an organization — such as a youth soccer club — you represent that you have authority to bind that organization to these terms.

These terms apply to all users including club administrators, coaches, and any other authorized users you grant access to.

Accounts and Access

You are responsible for maintaining the security of your account credentials and for all activity that occurs under your account. You must notify us immediately at hello@sportformiq.com if you become aware of any unauthorized access.

You are responsible for all coaches and administrators you add to your account. Their actions within the Service are your responsibility.

We reserve the right to suspend or terminate accounts that violate these terms.

Payment and Billing

One-time payments (Tryout and Camp tiers) are charged at the time of purchase and provide access for the stated period — typically two weeks from the date of purchase. One-time payments are non-refundable once access has been granted and roster data has been uploaded.

Annual subscriptions (Small Club, Club, and Academy tiers) are billed annually at the start of each subscription period. Annual subscriptions automatically renew unless cancelled at least 7 days before the renewal date.

All prices are in US dollars. Applicable sales tax is collected based on your billing address. Payments are processed by Stripe. We do not store your payment card information.

If a payment fails we will notify you by email. Access may be suspended for accounts with failed payments until the balance is resolved.

Your Data

You own your data. All player records, evaluation data, coaching notes, and decisions you create or upload belong to you and your club. We do not claim ownership of any content you submit to the Service.

We will not sell, share, or transfer your club's data to third parties without your explicit consent, except as required by law or as necessary to operate the Service (for example, payment processing via Stripe).

Each club's data is accessible only to authorized administrators and coaches within that club. No other club can access your data.

You may export your data at any time using the export features within the Service. Upon cancellation you have 30 days to export your data before it is permanently deleted from our systems.

Player and Medical Data

The Service allows you to upload and store information about youth players including names, dates of birth, and health and medical information such as allergies and medical conditions.

You are responsible for ensuring you have the legal right to collect, store, and process this information. This includes obtaining appropriate consent from players and their parents or guardians as required by applicable law.

You must ensure that health and medical information uploaded to the Service is accurate and kept up to date. SportFormIQ surfaces this information to authorized coaches as a safety tool — we are not responsible for the accuracy of information you provide.

For players under 13 years of age, you represent that you have obtained verifiable parental consent as required by the Children's Online Privacy Protection Act (COPPA) and any other applicable law before uploading their information to the Service.

We implement appropriate technical and organizational measures to protect player data. However, no software system is completely secure. You should maintain your own backup records of critical player information.

Acceptable Use

You agree to use the Service only for lawful purposes and in accordance with these terms. You agree not to:

• Upload data for players without appropriate authorization or parental consent
• Use the Service to harass, discriminate against, or harm any individual
• Attempt to gain unauthorized access to any part of the Service or another club's data
• Use the Service in any way that could damage, disable, or impair the platform
• Reverse engineer, copy, or resell any part of the Service
• Upload false, inaccurate, or misleading player information

We reserve the right to terminate access for any account that violates these terms without refund.

Privacy

Our full Privacy Policy is below. In summary: we collect only what we need to operate the Service, we do not sell your data, and you own everything you upload. See the Privacy Policy section below for complete details.

Limitation of Liability

The Service is provided "as is" without warranty of any kind. We make no guarantees about uptime, availability, or fitness for a particular purpose.

SportFormIQ's liability to you for any claims arising from use of the Service is limited to the amount you paid for the Service in the 12 months preceding the claim.

We are not liable for any indirect, incidental, special, or consequential damages including lost data, lost revenue, or any harm arising from decisions made using evaluation data generated through the Service. Evaluation data is a coaching tool — final decisions about players remain the sole responsibility of the club and its coaches.

We are not responsible for the accuracy, completeness, or appropriateness of evaluation data entered by coaches using the Service.

Cancellation

One-time plans expire automatically at the end of the access period. No cancellation is required.

Annual subscriptions can be cancelled at any time from your account settings. Cancellation takes effect at the end of the current billing period — you will retain access until that date. We do not provide prorated refunds for unused time on annual subscriptions.

To cancel or request account deletion contact us at hello@sportformiq.com. Upon cancellation you have 30 days to export your data.

Changes to These Terms

We may update these terms from time to time. When we make material changes we will notify you by email at the address associated with your account at least 14 days before the changes take effect.

Your continued use of the Service after the effective date of updated terms constitutes acceptance of the new terms. If you do not agree to the updated terms you should cancel your subscription before the effective date.

These terms are governed by the laws of the State of Ohio, United States, without regard to its conflict of law provisions.

What We Collect

Account information — name, email address, club name, and billing information when you create an account or make a purchase.

Player data — roster information you upload including player names, dates of birth, positions, and registration IDs from platforms such as GotSport, TeamSnap, or SportsEngine.

Health and medical data — allergies and medical conditions imported from your roster file. This data is flagged as sensitive and accessible only to authorized coaches within your club.

Evaluation data — coaching ratings, notes, and decisions entered by coaches using the Service.

Usage data — how you interact with the Service including pages visited, features used, and session duration. This is collected via Google Analytics on our marketing website and through server logs on the application.

Payment data — payment is processed by Stripe. We do not store credit card numbers or full payment details on our servers.

How We Use It

We use the information we collect to:

• Provide, operate, and improve the Service
• Process payments and send receipts
• Send service-related communications including support responses and product updates
• Surface player health and medical information to authorized coaches as a safety tool
• Generate evaluation reports and development history for players within your club
• Understand how the Service is used so we can improve it

We do not use your data for advertising. We do not sell personal data. We do not use player evaluation data to train AI models or share it with third parties for commercial purposes.

Player & Medical Data

Player data — including health and medical information — is treated with the highest level of sensitivity in our system.

Access controls: Player data is accessible only to administrators and coaches you have authorized within your club. Each club's data is completely isolated from all other clubs on the platform.

Medical data specifically: Allergies and health conditions are surfaced to authorized coaches at the point of evaluation as a safety tool. This information is never shared with other clubs, used for marketing, or included in any aggregated or anonymized datasets.

Children's data: SportFormIQ evaluates youth players, many of whom are under 13 years of age. We comply with the Children's Online Privacy Protection Act (COPPA). We do not knowingly collect personal information directly from children. All player data is submitted by clubs and their authorized administrators, who are responsible for obtaining appropriate parental consent.

Data minimization: We collect only the player fields necessary to operate the Service. We do not require or store Social Security numbers, financial information, or other sensitive personal data about players.

Data Sharing

We do not sell your data. We share data only in the following limited circumstances:

• Stripe — payment processing. Stripe's privacy policy applies to payment data.
• Formspree — interest form submissions from our marketing website. Email addresses and club information submitted via our contact form are processed by Formspree.
• Google Analytics — aggregated, anonymized usage data from our marketing website only. No player data is shared with Google.
• Legal requirements — we may disclose data if required by law, court order, or governmental authority.
• Business transfer — if SportFormIQ is acquired or merges with another company, your data may be transferred as part of that transaction. We will notify you before your data becomes subject to a different privacy policy.

We do not share player evaluation data, health data, or coaching notes with any third party under any circumstance other than those listed above.

Data Retention

We retain your data for as long as your account is active. If you cancel your account you have 30 days to export your data before it is permanently deleted from our systems.

Evaluation data and player records are retained for the duration of your subscription. We do not retain data beyond 30 days after account cancellation.

Payment records are retained as required by applicable tax law, typically seven years.

Your Rights

You have the right to:

• Access the data we hold about your club and players by exporting it at any time from within the Service
• Correct inaccurate data by editing player records within the Service
• Delete your data by cancelling your account and requesting deletion at hello@sportformiq.com
• Receive a copy of your data in a portable format using the CSV export feature

For players or parents who wish to request deletion of a specific player's data, requests should be directed to the club administrator who manages the account. The club administrator can delete individual player records from within the Service.
